Welcome New Patients

Welcome to our practice! We look forward to your initial visit. Our office is dedicated to caring for children from newborn to 21 years of age with issues regarding the gastrointestinal system, hepatobiliary system and nutritional problems. Our goal is to give you an expert and up-to-date evaluation and to educate you about any problems we may diagnose.

Initial Visit & New Patient Forms

The purpose of this visit is to begin a thorough evaluation concerning your childs medical issue. Your first visit will include a detailed medical history, physical examination and laboratory tests as necessary. At the conclusion of the initial visit, the significant findings will be summarized and the initial course of action will be outlined. We do not perform invasive procedures on the first visit. You are always encouraged to ask any questions. It is of the utmost importance to us that you understand the nature and treatment of your problem.

After making an appointment you will receive a new patient packet in the mail, in addition, you may also download the materials (English | Espanol). This packet will include directions, medical history forms as well as contact information forms. It is helpful if these forms are completed before you arrive.

Items to remember for your first visit:

  • Insurance Card
  • Any authorizations or scripts that your insurance may require
  • Any lab results or tests previously ordered by a physician which may relate to your visit
  • Your co-payment
  • Photo ID
